Warfighter Sports Chapter Grant Application

Disabled Sports USA is pleased to support Warfighter Sports chapter programming for disabled Veterans and members of the Armed Services taking place this summer (April 1 – Sept 30 2016). Deadline for submissions is February 21st (midnight).

Applications may be submitted for more than one program.  Please submit one application for each request – unless camps are directly related, i.e. a series.

Grants are available in the range of $1,000 – $10,000 to support camps, clinics, lessons, equipment and competitions that serve veterans and members of the armed forces with permanent physical disabilities (does not include PTSD as the sole diagnosis).

The larger grants will be awarded to the programs that have multiple aspects of the following priorities, including a comprehensive outreach and program plan.

*Please note that this application form cannot be saved and returned to.  It is recommended that you write the answers to longer questions in a separate file so that you have a saved document to return to in the event of technical difficulties.

Reporting Requirements – 

·       Expense report itemizing program expenses that meet or exceed grant award and are not being covered by other grants

·       Final grant report form

·       Complete list of participants with contact information (no exceptions)

·       Recognize sponsorship and provide success stories (including quotes and stories)
